SHOWCASE :This pt consulted Dr Inerdev Sahu of facebook fame, with a friend of his on 6/1/11.The pt is a farmer and a laborer. The conversation between them went like this. Dr: Whats your problem? What can I do for you? The patient opened his mouth and showed the doctor his tongue. Dr: Why don├óÔé¼Ôäót you go to the hospital for this problem, it is a bad condition. (The pt kept quiet) ok, you don├óÔé¼Ôäót have money to go to the hospital, tell me since when are you having this problem. (He had difficulty in talking but somehow he spoke in broken words as if the tongue was held by something) Pt: It started since 7-8 months back, when I had a boil, I went to local doctors and they told me it will get well but then the boil broke and then a hole was formed and it went on spreading. Dr :; don├óÔé¼Ôäót you have pain Pt: I had little pain and it used to subside by medicines (pain killers but now since 7-8 days the pain is not subsiding, it is not intolerable but it is not subsiding. Dr : Can you eat properly? Pt : I can├óÔé¼Ôäót eat from this side, if food particles touch over there is sudden pain and then the pain subsides immediately. Dr : Do you have any other problem somewhere else in the body.does this pain radiates to your ears or somewhere else Pt : Doctor sahib you just cure my tongue,I am ok everywhere,I don├óÔé¼Ôäót have any problem elsewhere. It was difficult for him to speak and he couldn├óÔé¼Ôäót give any answers so I asked him. Dr : Did you suffer from any other disease right from childhood Pt : When I was 17 years old I suffered from T.B Dr : Did you take proper treatment for that and do you remember which sided TB it was? PT:I took proper treatment for it and it was on the left side. Dr :Do u drink? Pt : Earlier I used to drink, but for the past 7 months I don├óÔé¼Ôäót drink( I didn't believe this part of his story as every alcoholic gives the same answer) What the treating physician saw, pt was an alcholic,chews tobacco too much,smokes bidis( an Indian rural version of cigarette) too much . He couldn├óÔé¼Ôäót open his mouth properly, due to fibrosis because of eating of gutka, ( perfumed tobacco and some preservatives, chemicals, known to cause cancer), he couldn├óÔé¼Ôäót protrude his tongue out, base of the tongue is eaten away, there is growth in the center of the tongue and tongue is covered with fungoid growths also, but there is no offensiveness yet ,which we generally get in cancer patients, (a peculiar odour which cannot be described but one who has handled cancer cases will know) The eyes of the patient were reddish yellow, the patient didn├óÔé¼Ôäót ask much about his disease so he knows what has happened to him, but there was no anxiety and fear, seeing the condition of the tongue there should be terrible pain, but this patient is not having that much pain, maybe he is insensible to pain due to his addictions.
